Form 64-8 Authorising your agent
If you decide to appoint an agent to act for you concerning tax matters, you or your agent should fill out this form and send it to HMRC at the address on the second page of the form.
Enter your name, the name of your business, and your agent’s registered name, e.g. our agent name is NLD Accountancy Limited. We have HMRC agent codes for self-assessment, PAYE, VAT and corporation tax. These codes should be entered on the form, otherwise authorisation may take longer.
In the second column of the form, enter the reference number(s) for the agent you wish to act for you with HMRC.
Highly advisable to keep a photocopy when you post it to HMRC.
